The S-303 form is used by Watchtower appointees known as circuit overseers to report their week of activity while visiting each congregation of Jehovah’s Witnesses.[1]

In November of 1987 circuit overseers received notice from Watchtower to use a newly revised S-303 form for reporting congregation activity during their bi-annual visit with congregations. These forms were dated 1/88. (The Watchtower organization revises these forms fairly often.)

Readers have asked what these forms look like and what sort of information Watchtower wants gathered during the visit of their appointed representative.
